The EFS is about understanding what you need.

The EFS is a set of nine questions and two activities and it takes about five minutes to complete. We encourage you to have another trusted family member or friend present, and invite their input on some of the questions. The activities involve drawing and walking. Four of the questions involve self-rating are best answered by the individual. Five other questions are facts that relate to past health care such as the number of medications. This is where the input of others is encouraged to ensure accuracy.

The EFS covers the following topics: cognition, general health status, functional independence, social support, medication use, nutrition, mood, continence, and mobility. Problems in any of these areas could prompt appropriate discussions about how to prevent problems and how to make improvements.
