Number of Hospital Admission
“In the past year, how many times have you been admitted to a hospital?”
This is an asterisked item, and as such, input from an accompanying person may be allowed depending on the result of the Clock Drawing Test.
Zero admissions receives a score of 0, one or two admissions receives a score of 1, and more than two visits receives a score of 2.
Hospital admission is defined as:
At least one day admitted under the care of a designated care team
Elective or acute
Intermediate care such as for patient rehabilitation
Active treatment in a long-term care or intermediate care facility
Conversely, the following do NOT count as hospital admission:
Visit to the emergency department
Pre-admission clinic
Day-surgery without an overnight stay
Non-acute care in a long-term care facility, respite or hospice
Transfer to a different hospital if contiguous with another admission that has already been counted

“In general, how would you describe your health?”
The Interviewer should provide the Interviewee with the Response Cue Sheet, to help Interviewee respond appropriately. Using the response of the Interviewee, the interviewer can then categorize and score the response as a 0, 1 or 2.
It is important to remember that this is a non-asterisked item, and as such only the interviewees response is used, not the impression of the accompanying person or the interviewer.
