Section 3: Functional Independence
Requirement of Help
“With how many of the following activities do you require help?”
The Interviewer should provide the Interviewee with the Response Cue Sheet, to help Interviewee respond appropriately.
This is an asterisked item, and as such, input from an accompanying person may be allowed depending on the result of the clock test. As this question is intended to assess the baseline functional status of the Interviewee, it is important that the response is accurate.
Here are some tips:
If there is a discrepancy between the Interviewee and the accompanying person, the Interviewer should use their best judgement
In case of recent change due to acute illness, the response should be based on the baseline of the interviewee outside of the hospital setting
If the interviewee recently recovered from acute illness, the response should be based on their new functional abilities outside of a hospital setting
The response should be based on the ability of the Interviewee, even if they don’t normally perform the task (i.e., if you had to do it for an extended period of time without help, could you do it?)
Ultimately the scoring rests on the judgement of the interviewer
A response of zero or one item receives a score of 0, two to four items receives a score of 1, and five to eight items receives a score of 2.
